Please be informed of the Clinic’s Covid-19 Phase 3 Policies as outlined below. The current policies have been created based on input from the CMTBC, the PHO, as well as insurance provider considerations and my individual conversations with you, my patients.

Phase 3 Survey

Two days prior to your scheduled appointment time you will be emailed a clinical survey. This survey must be completed online prior to your appointment.  You can review a copy of this survey in advance here.  If you do not agree to these terms, please do not book an appointment as you will not be seen. These policies will stay in effect until reviewed on September 1^st, 2021.

Masks

Masks still must be worn in the clinic space at all times.  Masks are still currently mandatory in all public spaces for individuals who have not received two doses of the Covid-19 vaccine. We will not be monitoring your vaccination status. To protect your privacy and offer the best possible care to my immunocompromised patients and family,  the mask policy in the clinic will remain in place.
